General Information: First Appointment
The intake appointment will last approximately 60 minutes. Please download all forms that are applicable to your family. Please complete the forms prior to the appointment so that the appointment is more beneficial. If you are unable to print and complete the forms, please notify Ms. Brown before the appointment. Please do not bring your child or their siblings to the first appointment, it is not beneficial for a child to be there as we discuss your concerns. It is beneficial if both parents can attend the first appointment.
In the case of divorce, please complete the additional forms that are applicable to that situation: Right to Seek Mental Health Treatment and Custody Dispute Contract. Please bring the latest legal decree regarding custody and the section that outlines who may seek and make decisions for mental health treatment . I am legally unable to treat your child or discuss your concerns without this legal documentation.

Cancellation Policy
Our clinicians schedule an exclusive time for you and or your child. If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session. Please be aware insurance does not reimburse for missed appointments. Please cancel Monday appointments by Friday.
